Consent Form As part of the entry process you must provide your consent and certain assurances to Alcohelp and Trading Standards. This includes consent for us to use an image or recording by you, reproduce the materials provided by you for the competition entry, and assurance that all performers in your film have consented to the use and recording of their performance. Please ensure that all contributors to your entry and performers sign a Consent Form; and that all forms are enclosed with your competition entry.   In the case of a person aged under 18 years of age, their parent and/or legal guardian should also sign this Consent Form where indicated below.
